What should I do if I fall ill when I’m away from home?

If you are ill while away from home or if you are not registered with a doctor but need to see one you can ask to be registered as a temporary patient at a nearby GP practice for up to three months. This process allows you to make use of a local practice but still remain a patient of your permanent GP practice at home. However, after three months, if you still wish to access this practice you will be required to permanently register.

If you wish to register with us as a temporary patient simply contact the practice directly.

When you register as a new patient, you will also be asked to fill out a medical questionnaire. This is because it could take considerable time for your medical records to be transferred to us. You can either fill out this form online and send it to us, or print it out and bring it with you to the practice on your first visit. To access the online form, please click on the link below.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
